Chronic Disease Management
 

Our chronic disease management section consists of a team of medical, nursing and allied health professionals who assist people with a chronic disease, and improve and maintain their health status and quality of life. We have been delivering care to people with chronic diseases for more than six years and our service is available to both the general public and designated private health fund members. This program is available throughout Australia.
Our service is designed to provide the required level of support to people to assist them better manage their health condition which is essential to achieving better health outcomes and an enjoyable lifestyle. Generally, people with a chronic disease have higher rates of hospital admission. We aim to effectively monitor people to limit escalations with their illness and reduce avoidable admissions.
This service includes:

  • a comprehensive health, lifestyle and functional assessment
  • coordination of services required, whether they be specialist area clinics ( eg. leg ulcer, falls and balance, sleep), medication management, medical liaison, social activities, community services or allied health care.
  • regular scheduled nurse telephonic monitoring
  • 24 hour access from anywhere in the world, to a triage nurse who has access to the person's current medical history and progress notes
  • educational information packages related to their chronic conditions
  • self management support services

The person's GP is always advised of our involvement and receives regular feedback on the progress of their patient. Should True Care become aware of any deterioration in a persons health, the person will be directed to make an appointment to see their GP and the GP will be notified.
