This breadth of knowledge and experience equips our company with the skill to identify risk factors and effectively assess and deliver care across various conditions.

Assessment outcomes need to be standardised, quantified and comparable if they are to have any positive impact upon health management. Therefore, we have invested significant time in developing appropriate assessment tools to deliver measurable care outcomes.

We use various electronic assessment tools for different purposes. For example we use the interRAI tool for chronic disease management due to its scientific and clinician based development and review process. True Care has adopted this instrumentation, added additional levels and designed a program that links the information to an electronic care plan. This approach has been followed to ensure consistency of assessment across staff and to achieve positive outcomes in the care of our clients.
